I, the undersigned, acknowledge that bouldering and rock climbing involve inherent risks, including but not limited to falls, contact with the climbing surface, and muscle or joint injury. I voluntarily choose to participate in bouldering activities at Earth Adventures Unlimited (Fort Wayne, Indiana).
I agree to inspect the facility and equipment before use and to refrain from using anything I believe to be unsafe. I accept full responsibility for my own safety and agree to follow all posted rules and staff instructions at all times.
In consideration of being permitted to use the bouldering gym, I hereby release, waive, and discharge Earth Adventures Unlimited, its owners, employees, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by me while participating in bouldering activities on the premises.
I confirm that I am 18 years of age or older (or that this waiver is being signed by my parent or legal guardian if I am a minor). I have read this waiver, understand its terms, and sign it voluntarily.
